Prior to the start of services, an assessment is conducted by a Gerontological Social Worker in order to determine the client's functioning capabilities, to identify their needs and to determine what services are required to assist them attain and maintain optimum independence. This assessment allows us to create a plan of care that addresses the needs identified. We then consult both clients and family members in order to reach an agreement on the most appropriate plan of care. Adjustments to the plan are made by close monitoring and communication with all parties involved.
